To make your visit as pleasant and informative as possible we ask that you read the following information and follow the instructions that apply to your situation.
- For Directions to our facility please click on the Contact Us link to your left.
- For convenience and faster service please print, fill out and bring with you the medical and insurance forms.
- All co-payments, private insurance and self-pay payments are expected at the time of service, unless other arrangements are made prior to your scheduled appointment.
- You are responsible for bringing any referrals that may be required by your insurance company. Referrals can be obtained by contacting your primary care physician. Your doctor can fax your referral to our office at 272-1260.
- If this is a No Fault or Worker's Compensation claim, you will need to bring the billing information with you to your appointment. This includes your employer name, telephone number and workers' compensation carrier. We also request that you bring private insurance information and any necessary referrals in the event that your No Fault or Worker's Compensation company denies your claim. If this is a No Fault visit, please sign the attached assignment of benefits form so that payment of your claim is not delayed.
- If you have had any x-rays, CT scans, MRIs, bone scans, etc., please bring the actual films with you to your appointment. Our Doctors feel that their personal review of your imaging studies is an essential part of your medical care. We can electronically access your images from the following facilities: Samaritan, Seton/St. Mary's, Image Care, Capital Imaging, Albany Advaned Imaging, and Capital Region Upright MRI.
- As a matter of courtesy, if you are unable to keep your appointment, please call within 24-hours of the scheduled time.
